Experimental Study of End-to-Side Portacaval Shunt

Abstract
After end-to side portacaval shunt in six experimental dogs, the author observed hematologic value, liver function test, blood biochemical test, portal pressure and histopathological findings of anastomotic stoma, liver, spleen and brain.
Results are as follows:
1. In hematologic findings, the postoperative mean values of RBC, MBC and hematocrit were increased but the platelets count was slightly decreased or normal.
2. In liver function test, impairement of hepatic function was found with increments of serum globulin, serum bilirubin, cholesterol, GOT, GPT, TTT, alkaline phosphatase and prothrombin time, and decrements of serum albumin and A/G ratio.
3. BUN and NPN were decreased after shunt operation but blood ammonia was not significantly changed at postoperative stage.
4. Mean postoperative portal pressure was 127.5 mm H©üO and not much increased compared with preoperative value showing 109.4 mm H©üO.
5. Histopathological changes of liver after shunting operation were mainly degeneration of hepatic cells and disarray of hepatic cell cords. The pathological abnormalities were not restored even at 12th week after the operation.
6. Microscopic findings of spleen showed sinusoidal congestion and hemorrhage, atrophy of white pulp, and hemosiderin pigmentation.
7. Histopathological findings of brain after operation revealed an increase of astrocytes, vascular congestion, edema and softening.
